THE FOLLOWING ITEMS MAY NOT ALL APPLY TO YOU. THIS LIST IS INTENDED TO PROVIDE SOME GUIDANCE WITH YOUR PERSONAL AND BUSINESS AFFAIRS.

* LIFE INSURANCE- Request claim forms from the company. Also request Change of Beneficiary@ forms for policies in which the deceased is named beneficiary.

*AUTO/HOMEOWNER INSURANCE- Notify respective agents. A premium refund may be due to you.

*HOSPITALIZATION/HEALTH INSURANCE- Notify carrier or employer if group coverage is provided by same. Consider a policy conversion if coverage terminates a benefit.

*REAL ESTATE, WILL AND ITEMS NOT JOINTLY TITLED- See Attorney.

*AUTO, RECREATION VEHICLES ETC.-See Secretary of State with title, AET test (if applicable) and copy of death certificate if item is jointly titled OR a spouse survives. If not jointly titled OR no spouse survives, see attorney.

*UTILITIES/TELEPHONE-It may be advisable to leave billing in the name of the deceased, if applicable. This is a security aid. 

*BANK, CREDIT UNION- Submit death certificate. Joint accounts become property of survivor. Certificates of Deposit may be converted without penalty (except current interest rate is applied to new CD). CONSIDER MAINTAINING ONE JOINT ACCOUNT FOR 60 DAYS TO ALLOW FOR DEPOSIT OF SMALL CHECKS RECEIVED AFTER DEATH.

*SAFE DEPOSIT BOX- Joint boxes are NOT sealed and are accessible to joint holder. If box is not joint, see bank manager.

*SOCIAL SECURITY- The Funeral Home will send in a Statement of Death Form SSA 72, which will initiate your claim. Other benefits May be due you. For any specific questions you may have call 1-800-772-1213 or 517-773-5411.
